Governance & Guardrails (gap 1.6). Publish the rules that already exist informally: what AI may touch, what it may not, and who decides exceptions. The team is guessing, and guessing conservatively costs you adoption.

Oversight & Verification (gap 1.3). Leaders believe review happens; the team reports it mostly does not. Define the review gates by consequence and make them visible.

Culture & Psychological Safety (gap 1.5). The team does not believe disclosure is safe. Until unapproved AI use can be discussed without consequence, every other number in this report is understated.
